    A first-year student at a university, college, or high school.

    - Original High School Card (Form 138) - Student Permanent Record (Form 137) - Certificate of Good Moral Character - Original Copy of PSA Birth Certificate - Drug Test Result - Four (4) 2x2 and 1x1 ID Pictures

  • Transferee:

    A student who comes to a university or school after having begun his or her course of study at a different university or school.

    - Official Transcript of Records - Certificate of Honorable Dismissal - PSA Birth Certificate - Drug Test Result - Four (4) 2x2 and 1x1 ID Pictures

  • ALS Graduate:

    Alternative Learning System - a parallel learning system in the Philippines that provides a practical option to the existing formal education. When one does not have or cannot access formal education in schools, ALS is an alternate or substitute.

    - ALS Certificate - ALS Diploma - ALS Rating - Certificate of Good Moral Character - PSA Birth Certificate - Drug Test Result - Four (4) 2x2 and 1x1 ID Pictures
